Last Known Activity Lt. Clearwaters was killed during his rescue by the crew of UH-1H (#68-15589) from the 227th Aviation Battalion, flying out of Phuoc Vinh Army Air Base, Phuoc Than Province. The aircraft was brought down by small arms and B-40 fire, killing all on board with the exception of CWO2 Cleary who was blown clear of the crash and survived.
