INTERESTING OFFICIAL DOCUMENT - as it contains incorrect information.
The plane described in the document is the Green Hornet that Phillips and Zamperini were flying. However, the Green Hornet was a B-24D. The plane did belong to the 42nd Bomb Sq and it was being used for Search & Rescue near Palmyra Island.
Zamperini was not the co-pilot; he was the bombardier. The co-pilot was 1st Lt Charlton H. Cupernell (KIA).
The crew manifest is correct as best determined. All names are not spelled correctly. The unidentified person is still a mystery and we can only assume the Army or Air Force today knows who this person is - can't say missing forever. Possibly there never was an unidentified person on board.
