Last Known Activity

CW2 Vaquera was killed as the result of a collision of an Air Force FAC O-2A, piloted by 1st Lt. Henderson,  and his CH-47C (tail number 67-18513) near Chu Lai, about 5 miles NE of Tam Ky, Vietnam, while en route to a visual reconnaissance mission. Crew of both aircraft were lost.
