WO1 Patrick O'Brien was the co-pilot of UH-1H (hull number 67-17587:)that crashed killed all 4 crew and 2 passengers. 1 passenger survived. The aircraft crashed along a river when it was attempting to avoid a fishing tower and the tailboom struck a sandbar.

WO1 O'Brien was interred in Arlington National Cemetery, Arlington, Arlington County, Virginia, Plot: Sec: 41, Site: 1887-1.
