Last Known Activity
On 20 February 1971, SP4 Levulis was the medic aboard a UH-1H (#69-15273) that crashed and burned in dense fog at the Khe Sanh airfield, Huong Hoa District, northern Quang Tri Province.

All crew and passengers suffered mortal injuries:
WO John V. Rauen, (P) KIA
WO John M. Souther, (CP) (died of injuries 26 Feb)
SP4 Dennis E. Gilliland, (CE) died of injuries
SP4 John J. Levulis, (Medic) died of injuries

SP4 Levulis suffered severe burns and was pulled from the aircraft.
