On 25 February, WO1 Patrick Erb was the co-pilot of a UH-1H (tail number 68-15403) that crashed in an landing zone near LZ Scotch during Lam Son 719 Pilot was WO1 Steven R. Burch who also later died form lung injuries.

His unit was the 174th Assault Helicopter Company, 14th Combat Aviation Battalion..

He was severely burned, and died of complications including pneumonia in a military hospital in Japan on 15 March 1971..
