From Form DA 2496 Individual Casualty as the Result of Hostile Action 16 August 1968
 
Individual died from wounds received while driver of a military vehicle on an escort mission when the vehicle came under a hostile anti-tank round attack.  Individual was admitted to a military hospital, placed on the SI (serious injury) list, evacuated to Japan, and placed on the VSI (vary serious injury) list where he expired shortly thereafter.
 
Date of incident in Vietnam not mentioned.  Location of injuries was in Phuoc Long Province, Republic of South Vietnam.
