On 11 July 1967, 1st Lt. Dwyer was a pilot of a UH-1D (#63-08844) that departed a firebase in the Central Highlands at approximately 1000hrs for the purpose of reconning the site of a scheduled air assault. The weather was overcast and cloudy with monsoon season rain. The aircraft was flying around the Kontum area when it lost its engine. It was destroyed on impact in mountainous terrain.
