CW2 Concannon was serving as a pilot when he was killed when the UH-1H (tail number 68-16554) crashed 2 miles out on radar approach to Phu Bai after night MEDEVAC i monsoon weather.  The aircraft flew too low and the wires entangled into the canopy, causing the aircraft to crash.

Four crewmen and three passengers were killed in the loss.
